Résumé

Objectif Dans le contexte canadien, nous avons cherché à examiner la relation entre les ménages ayant des enfants/jeunes autistes et le revenu des ménages. Plan de l'étude Nous avons utilisé les données de l'Enquête canadienne sur la santé des enfants et des jeunes (ECSEJ) de 2019 afin d'analyser les ménages ayant un enfant/jeune âgé de 1 à 17 ans. Un appariement par score de propension a été utilisé pour associer les dossiers d'enfants/jeunes ayant un diagnostic d'autisme déclaré à ceux d'enfants/jeunes n'en ayant pas. Nous avons utilisé une régression linéaire pour les résultats continus (p. ex., le revenu total des ménages) et une régression de Poisson pour les résultats binaires (p. ex., un faible revenu des ménages). Toutes les analyses ont été ajustées pour tenir compte de la corrélation entre les paires appariées. Principal critère de jugement Revenu annuel total de tous les membres du ménage. Critères de jugement secondaires Faible revenu des ménages; statut de parent seul ou de revenu unique; et présence d'au moins un parent ne travaillant pas ou absent du travail au cours de la semaine précédente. Résultats Parmi un total de 39 951 dossiers de l'ECSEJ, nous avons identifié une cohorte de 815 enfants/jeunes autistes. Les caractéristiques de la cohorte appariée étaient bien équilibrées. Les ménages ayant un enfant/jeune autiste avaient un revenu annuel moyen des ménages inférieur (différence moyenne : 16 489 $ CA; IC à 95 % : de 6 384 $ CA à 27 149 $ CA) à celui des ménages appariés sans enfant/jeune autiste. Les ménages ayant un enfant/jeune autiste étaient également 26 % plus susceptibles d'être classés comme ayant un faible revenu des ménages (risque relatif [RR] 1,26; IC à 95 % : de 1,17 à 1,35) et 20 % plus susceptibles de dépendre d'un revenu unique (RR 1,20; IC à 95 % : de 1,10 à 1,33) que les ménages sans enfant/jeune autiste. Conclusions Comparativement aux ménages sans enfant/jeune autiste, ceux ayant un enfant/jeune autiste font souvent face à davantage de difficultés économiques, notamment un revenu des ménages inférieur et un risque accru d'insécurité alimentaire. Les ménages ayant un enfant/jeune autiste sont plus susceptibles de dépendre d'un revenu unique.

Objective Within the Canadian context, we sought to examine the relationship between households with autistic children/youths and household income. Design We used data from the 2019 Canadian Health Survey on Children and Youth (CHSCY) to analyse households with a child/youth aged 1–17. Propensity-score matching was used to pair records for children/youths with a reported autism diagnosis to those without. We used linear regression for continuous outcomes (eg, total household income) and Poisson regression for binary outcomes (eg, low household income). All analyses were adjusted for the correlation between matched pairs. Primary outcome measure Total annual income of all household members. Secondary outcome measures Low household income; single-parent or single-income status; and whether at least one parent was not working or absent from work during the past week. Results Among a total of 39 951 CHSCY records, we identified a cohort of 815 autistic children/youths. The characteristics of the matched cohort were well-balanced. Households with an autistic child/youth had a mean annual household income that was lower (mean difference: $C16 489; 95% CI $C6384 to $C27 149) compared with matched households without an autistic child/youth. Households with an autistic child/youth were also 26% more likely to be classified as having a low household income (Relative risk (RR) 1.26; 95% CI 1.17 to 1.35) and 20% more likely to rely on a single income (RR 1.20; 95% CI 1.10 to 1.33) compared with households without an autistic child/youth. Conclusions Compared with households without an autistic child/youth, those with an autistic child/youth often face more economic challenges, including lower household income and greater risk of food insecurity. Households with an autistic child/youth are more likely to rely on a single income.
