Résumé

OBJECTIF : Cette étude est la première d'une série de quatre investigations empiriques visant à élaborer des ensembles de base de la Classification internationale du fonctionnement, du handicap et de la santé (CIF) pour le trouble du spectre de l'autisme (TSA). L'objectif était d'utiliser une approche de revue systématique pour identifier, dénombrer et relier les concepts d'aptitude fonctionnelle et de handicap employés dans la littérature scientifique sur le TSA à la nomenclature de la CIF-EA (version de la CIF pour enfants et adolescents, couvrant l'ensemble de la durée de vie). MÉTHODES : Des recherches systématiques portant sur des études de résultats concernant le TSA ont été réalisées dans Medline/PubMed, PsycINFO, ERIC et Cinahl, et les concepts pertinents d'aptitude fonctionnelle et de handicap ont été extraits des études incluses. Ces concepts ont ensuite été reliés à la CIF-EA par deux chercheurs indépendants à l'aide d'une procédure de liaison standardisée. De nouveaux concepts ont été extraits des études jusqu'à ce que la saturation des catégories de la CIF-EA identifiées soit atteinte. RÉSULTATS : Soixante et onze études ont été incluses dans l'analyse finale et 2 475 concepts significatifs contenus dans ces études ont été reliés à 146 catégories de la CIF-EA. Parmi celles-ci, 99 catégories ont été considérées comme les plus pertinentes pour le TSA (c'est-à-dire identifiées dans au moins 5 % des études), dont 63 étaient liées aux activités et à la participation, 28 aux fonctions organiques et 8 aux facteurs environnementaux. Les cinq catégories les plus fréquemment identifiées étaient les interactions interpersonnelles de base (51 %), les fonctions émotionnelles (49 %), les interactions interpersonnelles complexes (48 %), les fonctions attentionnelles (44 %) et les fonctions mentales du langage (44 %). CONCLUSION : La grande variété de catégories de la CIF-EA identifiées dans cette étude reflète l'hétérogénéité des différences fonctionnelles observées dans le TSA, tant en ce qui concerne le handicap que l'exceptionnalité, et souligne la valeur potentielle de la CIF-EA en tant que cadre permettant de saisir le fonctionnement d'un individu dans toutes les dimensions de la vie. Les résultats actuels, combinés à trois études préparatoires supplémentaires (enquête auprès d'experts, groupes de discussion et étude clinique), fourniront la base scientifique pour définir les ensembles de base de la CIF pour le TSA, destinés à une utilisation polyvalente dans la recherche fondamentale et appliquée ainsi que dans la pratique clinique quotidienne du TSA.

OBJECTIVE: This study is the first in a series of four empirical investigations to develop International Classification of Functioning, Disability and Health (ICF) Core Sets for Autism Spectrum Disorder (ASD). The objective was to use a systematic review approach to identify, number, and link functional ability and disability concepts used in the scientific ASD literature to the nomenclature of the ICF-CY (Children and Youth version of the ICF, covering the life span). METHODS: Systematic searches on outcome studies of ASD were carried out in Medline/PubMed, PsycINFO, ERIC and Cinahl, and relevant functional ability and disability concepts extracted from the included studies. These concepts were then linked to the ICF-CY by two independent researchers using a standardized linking procedure. New concepts were extracted from the studies until saturation of identified ICF-CY categories was reached. RESULTS: Seventy-one studies were included in the final analysis and 2475 meaningful concepts contained in these studies were linked to 146 ICF-CY categories. Of these, 99 categories were considered most relevant to ASD (i.e., identified in at least 5% of the studies), of which 63 were related to Activities and Participation, 28 were related to Body functions, and 8 were related to Environmental factors. The five most frequently identified categories were basic interpersonal interactions (51%), emotional functions (49%), complex interpersonal interactions (48%), attention functions (44%), and mental functions of language (44%). CONCLUSION: The broad variety of ICF-CY categories identified in this study reflects the heterogeneity of functional differences found in ASD--both with respect to disability and exceptionality--and underlines the potential value of the ICF-CY as a framework to capture an individual's functioning in all dimensions of life. The current results in combination with three additional preparatory studies (expert survey, focus groups, and clinical study) will provide the scientific basis for defining the ICF Core Sets for ASD for multipurpose use in basic and applied research and every day clinical practice of ASD.
