Résumé

Objectif Les personnes trans sont confrontées à des risques accrus pour la santé et à des difficultés socio-environnementales, qui influencent leur engagement dans des comportements protecteurs pour la santé (p. ex., exercice et nutrition). Malgré ces difficultés, il existe une lacune importante dans la compréhension des expériences spécifiques en matière d'alimentation et d'exercice des adultes trans australiens, y compris les obstacles aux comportements favorables à la santé et les expériences de soins de santé. Cette étude vise à combler cette lacune en explorant ces expériences, afin d'éclairer des interventions ciblées et des pratiques de soins de santé visant à améliorer les résultats de santé. Méthode Nous avons mené des entretiens semi-structurés avec 25 participants afin d'explorer leurs expériences relatives aux comportements alimentaires et d'exercice (12 personnes transgenres et 13 dans le domaine des soins de santé ; six appartenaient aux deux catégories). Les participants ont été recrutés par l'intermédiaire de services de santé et communautaires, les entretiens ont été enregistrés et transcrits, et les données ont été analysées au moyen d'une analyse thématique réflexive. Résultats Les comportements alimentaires et d'exercice des adultes trans semblent être influencés positivement (p. ex., favorables à la santé) et/ou négativement (p. ex., nocifs) par leurs expériences liées au genre et leur contexte socio-environnemental. Les comportements favorables à la santé comprenaient l'exercice individuel et social au sein de la communauté ainsi que des pratiques alimentaires intentionnelles visant à soutenir la santé, tandis que les comportements potentiellement nocifs comprenaient l'exercice compulsif et l'alimentation restrictive. L'analyse a permis d'identifier trois thèmes qui façonnaient la motivation et les comportements des personnes trans : (1) l'affirmation de genre, (2) la neurodivergence et (3) les influences environnementales. Discussion Les résultats soulignent que les comportements alimentaires et d'exercice chez les personnes trans sont façonnés par des facteurs complexes, notamment l'expression de genre et la dysphorie de genre, la neurodivergence concomitante et les désavantages systémiques. Ces dynamiques contribuent de manière importante à la forte prévalence de comportements nocifs, notamment l'alimentation désordonnée et les troubles des conduites alimentaires dans cette population, et nécessitent l'intégration d'approches affirmant le genre et centrées sur la personne dans les soins de santé et les politiques publiques. L'évaluation et la prise en charge actuelles des comportements d'alimentation désordonnée et d'exercice ne répondent pas aux besoins spécifiques des personnes trans et soulignent la nécessité de stratégies inclusives et efficaces pour prévenir et traiter les comportements d'exercice et d'alimentation désordonnés.

Objective Trans individuals face elevated health risks and socio-environmental challenges, influencing their engagement in health-protective behaviors (e.g. exercise and nutrition). Despite these challenges, there is a significant gap in understanding the specific eating and exercise experiences of Australian trans adults, including barriers to healthy behaviors and healthcare experiences. This study aims to address this gap by exploring these experiences, informing targeted interventions and healthcare practices to improve health outcomes.Method We conducted semi-structured interviews with 25 participants to explore their experiences with eating and exercise behaviors (12 transgender individuals and 13 healthcare; six were from both categories). Participants were recruited from health and community services, interviews were recorded and transcribed, and data analyzed using reflexive thematic analysis.Results The eating and exercise behaviors of trans adults appear to be influenced positively (e.g. health promoting) and/or negatively (e.g. harmful) by their gendered experiences and socio-environmental context. Healthy behaviors included individual- and social-based exercise within the community and intentional eating practices to support health, whereas potentially harmful behaviors included compulsive exercise and restrictive eating. Analysis resulted in identification of three themes that shaped the motivation and behaviors of trans folk: (1) gender affirmation, (2) neurodivergence, and (3) environmental influences.Discussion The findings underscore that eating and exercise behaviors among trans people are shaped by complex factors including gender expression and gender dysphoria, co-occurring neurodivergence, and systemic disadvantages. These dynamics contribute significantly to the high prevalence of harmful behaviors, including disordered eating and eating disorders in this population, and necessitate the integration of gender-affirming and person-centered approaches in healthcare and public policy. The current assessment and management of disordered eating and exercise behaviors fail to address the specific needs of trans individuals and highlights the need for inclusive and effective strategies to prevent and treat disordered exercise and eating behaviors.
