Résumé
Le syndrome de l'X fragile (FXS), principale cause héréditaire de déficience intellectuelle, présente un taux de concomitance avec le trouble du spectre de l'autisme (TSA) estimé à environ 60 %. L'apparition et les taux de développement moteur dans le FXS sont plus lents par rapport au développement neurotypique, et davantage encore dans le contexte d'un FXS + TSA concomitant. Les données existantes suggèrent que ces différences sont susceptibles d'affecter la communication, mais ce processus développemental, ou la manière dont il varie dans le contexte d'un TSA concomitant, demeure inconnu dans le FXS. Nous avons cherché à délimiter les associations développementales entre les capacités motrices précoces et leur taux de développement de 9 à 60 mois, d'une part, et les résultats communicationnels, d'autre part, chez 51 enfants atteints de FXS, dont 28 présentaient un TSA concomitant. Nous avons également cherché à identifier les variations de ces associations développementales dans le contexte d'un TSA concomitant. Les résultats ont mis en évidence une variabilité au sein du syndrome dans ces associations développementales en fonction de la présence d'un TSA concomitant. La motricité fine s'est avérée être un prédicteur robuste de la communication réceptive, indépendamment de la présence d'un TSA concomitant, mais nous avons identifié des différences entre le FXS avec et sans TSA dans l'association entre certains aspects du développement moteur et les résultats expressifs. Les résultats fournissent des éléments en faveur de processus développementaux différentiels dans le contexte d'un TSA concomitant, avec des implications pour une intervention développementale en temps opportun. Résumé destiné au grand public Le syndrome de l'X fragile (FXS), principale cause héréditaire de déficience intellectuelle, présente un taux de concomitance avec le trouble du spectre de l'autisme (TSA) estimé à environ 60 %. Les enfants atteints de FXS connaissent une acquisition retardée et un développement plus lent de capacités motrices essentielles, ce qui se produit dans une mesure encore plus importante chez les enfants présentant à la fois un FXS et un TSA. De nombreuses études ont démontré que les capacités motrices constituent des compétences fondamentales liées aux résultats communicationnels ultérieurs dans le développement neurotypique, ainsi que dans le contexte du TSA. Toutefois, ces associations restent non examinées dans le FXS, ou le FXS + TSA. Dans cette étude, nous avons cherché à déterminer les associations entre les compétences motrices précoces et leur taux de développement, d'une part, et les résultats communicationnels dans le FXS, d'autre part. En outre, nous avons examiné si ces associations variaient dans le contexte d'un FXS + TSA concomitant. Les résultats ont révélé, au sein du FXS, des variations dans le contexte d'un TSA concomitant entre certains aspects du développement moteur et les résultats communicationnels, mais une cohérence au sein du FXS entre d'autres aspects. Les résultats fournissent des éléments en faveur d'une variabilité des processus et des résultats développementaux dans le FXS dans le contexte d'un TSA concomitant et offrent des implications pour l'intervention.
