Résumé

De nombreux enfants présentant un trouble du spectre de l'autisme (TSA) manifestent des symptômes associés à un dysfonctionnement du système nerveux autonome (SNA), indiquant une faible flexibilité psychophysiologique. Il est suggéré que les symptômes du TSA sont associés à des anomalies généralisées du système nerveux central, y compris des structures et des réseaux impliqués dans la régulation descendante du SNA. La stimulation magnétique transcrânienne répétitive (rTMS) a été suggérée comme une thérapie possible pour cibler les déficits de régulation du SNA dans le TSA. Dans la présente étude, nous avons utilisé une neuromodulation fondée sur la rTMS au niveau du cortex préfrontal dorsolatéral (DLPFC) afin de réduire l'activation sympathique et d'augmenter l'activité parasympathique chez des enfants présentant un TSA. Dans une étude portant sur 27 enfants autistes, nous avons administré une rTMS hebdomadaire à 0,5 Hz bilatéralement au niveau du DLPFC, avec enregistrement concomitant de l'activité autonome. L'analyse statistique des indices de variabilité de la fréquence cardiaque (VFC) dans les domaines temporel et fréquentiel, ainsi que du niveau de conductance cutanée (SCL), a révélé une forte régression linéaire pour la plupart des mesures de VFC et de SCL. Plusieurs scores d'évaluation comportementale parentale se sont améliorés après la TMS et ont montré une corrélation avec les résultats autonomes ; en particulier, les indices parasympathiques de VFC étaient négativement corrélés aux comportements répétitifs et stéréotypés, tandis que les indices d'activation sympathique présentaient une corrélation positive avec ces mêmes comportements. L'article examine les mécanismes neurobiologiques potentiels impliqués dans l'équilibre autonome après TMS et dans l'amélioration des comportements aberrants.

Many children with autism spectrum disorder (ASD) exhibit symptoms associated with autonomic nervous system (ANS) dysfunction indicative of low psychophysiological flexibility. It is suggested that ASD symptoms are associated with generalized abnormalities in the central nervous system, including structures and networks involved in the top-down regulation of the ANS. Repetitive transcranial magnetic stimulation (rTMS) has been suggested as a possible therapy to target ANS regulation deficits in ASD. In the current study we used neuromodulation based on rTMS over the dorsolateral prefrontal cortex (DLPFC) to reduce sympathetic arousal and increase parasympathetic activity in children with ASD. In a study on 27 children with autism we administered weekly 0.5 Hz rTMS bilaterally over the DLPFC with concurrent recording of autonomic activity. Statistical analysis of time and frequency domain heart rate variability (HRV) indices and skin conductance level (SCL) revealed a strong linear regression of most HRV and SCL measures. Several parental behavioral rating scores improved post-TMS and showed a correlation with autonomic outcomes, in particular parasympathetic indices of HRV negatively correlated with repetitive and stereotyped behaviors, while sympathetic arousal indices showed positive correlation with the same behaviors. The paper discusses potential neurobiological mechanisms involved in post-TMS autonomic balance and aberrant behavior improvements.
