Résumé

Résumé Contexte : Les troubles anxieux comorbides sont fréquents chez les enfants présentant des troubles du spectre de l'autisme (TSA), mais seule une minorité reçoit un traitement adéquat de l'anxiété. Il a été démontré que la thérapie cognitivo-comportementale (TCC) est efficace dans le traitement des troubles anxieux. Les objectifs de la présente étude pilote étaient de tester la faisabilité du programme de TCC « Moins de stress » pour les troubles anxieux comorbides chez des enfants présentant un TSA et d'explorer si une amélioration des résultats diagnostiques relatifs aux troubles anxieux et des symptômes d'anxiété était observée après le traitement. Méthodes : Les participants étaient dix enfants diagnostiqués avec un TSA et des troubles anxieux (huit garçons, âge moyen = 9,5 ans, intervalle 8 - 12 ans). Le programme « Less Stress » comprend trois mois de séances de traitement hebdomadaires suivis de trois séances de rappel mensuelles. Cinq thérapeutes ont participé. Un entretien diagnostique semi-structuré standardisé avec les mères a été utilisé pour évaluer les troubles comorbides. Les symptômes d'anxiété chez les enfants ont été mesurés à l'aide de la Revised Child Anxiety and Depression Scale (RCADS). Résultats : Les thérapeutes ont trouvé le manuel facile à utiliser, mais des adaptations étaient nécessaires, notamment des séances plus courtes en raison d'un trouble déficitaire de l'attention avec hyperactivité comorbide fréquent (n = 7). Les participants ont trouvé le programme utile et les parents ont noté qu'ils avaient appris des méthodes qu'ils pourraient continuer à utiliser après la fin du programme. Huit enfants sur dix ont achevé le traitement. Sept des huit participants ayant achevé le traitement ont bénéficié du programme. Cinq de ces sept enfants ne présentaient plus aucun trouble anxieux, tandis que deux présentaient moins de troubles anxieux. Au niveau du groupe, une réduction moyenne significative des symptômes d'anxiété (RCADS) a été observée après le traitement. Conclusion : Les thérapeutes ont considéré le programme « Moins de stress » comme une intervention réalisable dans un échantillon d'enfants présentant un TSA et une anxiété comorbide. La réduction significative de l'anxiété après le traitement est prometteuse, mais une réplication dans une étude plus vaste et plus rigoureuse est nécessaire pour examiner l'efficacité de l'intervention.

Abstract Background: Comorbid anxiety disorders are prevalent in children with autism spectrum disorders (ASD), but only a minority receives adequate treatment for anxiety. Cognitive behavioral therapy (CBT) has been shown to be effective in treating anxiety disorders. The objectives of the present pilot study were to test the feasibility of the CBT program “Less stress” for comorbid anxiety disorders in children with ASD and explore whether an improvement in diagnostic outcomes for anxiety disorders and symptoms of anxiety was found after treatment. Methods: Participants were ten children diagnosed with ASD and anxiety disorders (eight boys, mean age = 9.5 years, range 8 - 12 years). The “Less Stress” program includes three months of weekly treatment sessions followed by three monthly booster sessions. Five therapists participated. A standardized semi-structured diagnostic interview with the mothers was used to assess comorbid disorders. Child anxiety symptoms were measured with the Revised Child Anxiety and Depression Scale (RCADS). Results: The therapists found the manual easy to use but adaptations were necessary, particularly shorter sessions due to frequent (n = 7) comorbid Attention-Deficit/Hyperactivity Disorder. The participants found the program useful and the parents noted that they had learned methods they could continue using after the end of the program. Eight of ten children completed the treatment. Seven of the eight completers benefited from the program. Five of those seven children were free from all anxiety disorders, while two had fewer anxiety disorders. On a group level, a significant mean reduction of anxiety symptoms (RCADS) was found after treatment. Conclusion: The therapists found the “Less stress” program to be a feasible intervention in a sample of children with ASD and comorbid anxiety. The significant reduction of anxiety after treatment is promising, but a replication in a larger and more rigorous study is needed to investigate the effectiveness of the intervention.
