Résumé

Contexte : Les personnes autistes présentent des niveaux élevés de mauvaise santé mentale et un risque accru de suicide tout au long de la vie. Pourtant, les personnes autistes signalent des difficultés à communiquer avec les professionnels de santé et à accéder à un éventail de services de santé. Parallèlement, les professionnels de la santé mentale d'autres pays font état de liens entre leur confiance lorsqu'ils travaillent avec des patients autistes et le degré de connaissances et de formation sur l'autisme auxquels ils peuvent accéder. Méthodes : Nous avons cherché à examiner quels facteurs aidaient ou entravaient les collègues irlandais de la santé mentale lorsqu'ils travaillaient avec des usagers autistes des services de santé. Une enquête en ligne utilisant des mesures quantitatives et qualitatives a été diffusée auprès de psychiatres membres du College of Psychiatrists of Ireland, tant en formation qu'au niveau de consultant, d'avril 2021 à avril 2022. Résultats : Les connaissances sur l'autisme étaient élevées parmi les psychiatres (n = 140), mais les scores d'auto-efficacité étaient variables, en particulier en ce qui concerne les parcours de soins. L'auto-efficacité était meilleure chez les psychiatres dont la file active comprenait des enfants et des jeunes ou des personnes présentant des déficiences intellectuelles concomitantes. Trois thèmes qualitatifs clés ont émergé concernant les capacités et la formation des professionnels de la santé mentale, les moyens d'améliorer la prestation de services de santé mentale pour les personnes autistes, ainsi que le besoin crucial de co-création et de soins affirmatifs de la neurodiversité. Conclusions : L'étude a mis en évidence des défis systémiques et professionnels critiques dans la fourniture de soins de santé mentale aux personnes autistes en Irlande. Nous formulons des recommandations visant à réduire ces défis et à permettre le développement de soins inclusifs fondés sur des données probantes destinés aux personnes autistes.

Background: Autistic people have high levels of mental ill-health and an increased risk of suicide across the lifespan. Yet autistic people report difficulties communicating with healthcare professionals and accessing a range of healthcare services. At the same time, mental healthcare workers in other countries are reporting links between confidence when working with autistic patients and the degree of autism knowledge and training they can access. Methods: We sought to examine what factors helped or hindered Irish mental healthcare colleagues when working with autistic healthcare service users. An online survey using quantitative and qualitative metrics was circulated among psychiatrists who are members of the College of Psychiatrists of Ireland, both in training and at consultant level, from April 2021 to April 2022. Results: Knowledge of autism was high among psychiatrists (n = 140), but self-efficacy scores were variable, particularly in relation to care pathways. Self-efficacy was better among psychiatrists with caseloads of children and youth or individuals with co-occurring intellectual disabilities. Three key qualitative themes emerged relating to capacity and training of mental health professionals, ways to improve mental health services provision for autistic individuals and also the critical need for co-creation and neurodiversity affirmative care. Conclusions: The study highlighted critical systemic and professional challenges in providing mental health care to autistic people in Ireland. We provide recommendations for reducing these challenges and for enabling the development of inclusive, evidenced-based care to autistic individuals.
