Résumé

Cette étude examine l'impact potentiel du conflit et de la cohésion familiaux, ainsi que du soutien des pairs et du harcèlement, sur les enfants atteints de trouble du spectre de l'autisme (TSA). Bien que de tels impacts aient été établis pour un éventail de troubles de l'enfance autres que les TSA, ces résultats pourraient ne pas être généralisables aux enfants atteints de TSA en raison de problèmes spécifiques de prise de perspective, de compréhension des émotions d'autrui, de rigidité cognitive et de raisonnement social. Une approche de construction de modèles structurels a été utilisée pour tester dans quelle mesure les variables familiales et liées aux pairs affectaient directement ou indirectement les TSA par l'intermédiaire de l'anxiété et de la dépression de l'enfant. L'échantillon (N = 322) était composé de parents d'enfants atteints de TSA orientés vers deux cliniques spécialisées. L'échantillon comprenait des parents d'enfants présentant un trouble autistique (n = 76), un trouble d'Asperger (n = 188), un trouble envahissant du développement non spécifié (n = 21), ainsi que d'enfants avec un diagnostic autre qu'un TSA ou sans diagnostic (n = 37). Les parents ont rempli des questionnaires en ligne via un site web sécurisé. Les principaux résultats étaient que l'anxiété et la dépression, ainsi que la symptomatologie des TSA, étaient significativement liées, et que le conflit familial était davantage prédictif de la symptomatologie des TSA que les influences familiales positives et celles des pairs. Les résultats soulignent l'utilité d'élargir les interventions afin d'inclure la gestion des conflits pour les couples, même lorsque le conflit et la détresse familiale sont faibles. Des recherches supplémentaires sont nécessaires sur les significations potentiellement différentes de la cohésion et du conflit familiaux pour les enfants atteints de TSA par rapport aux enfants sans TSA.

This study examines the potential impact of family conflict and cohesion, and peer support/bullying on children with autism spectrum disorder (ASD). While such impacts have been established for a range of non-ASD childhood disorders, these findings may not generalize to children with ASD because of unique problems in perspective-taking, understanding others' emotion, cognitive rigidity, and social reasoning. A structural model-building approach was used to test the extent to which family and peer variables directly or indirectly affected ASD via child anxiety/depression. The sample (N = 322) consisted of parents of children with ASD referred to two specialist clinics. The sample contained parents of children with Autistic Disorder (n = 76), Asperger Disorder (n = 188), Pervasive Disorder Not Otherwise Specified (n = 21), and children with a non-ASD or no diagnosis (n = 37). Parents completed questionnaires on-line via a secure website. The key findings were that anxiety/depression and ASD symptomatology were significantly related, and family conflict was more predictive of ASD symptomatology than positive family/peer influences. The results point to the utility of expanding interventions to include conflict management for couples, even when conflict and family distress is low. Further research is needed on the potentially different meanings of family cohesion and conflict for children with ASD relative to children without ASD.
