Résumé

OBJECTIF : La thérapie familiale centrée sur les troubles du comportement alimentaire (FT-ED) est la principale intervention ambulatoire pour les adolescents atteints d'anorexie mentale. Les personnes autistes rapportent de moins bonnes expériences de traitement des troubles du comportement alimentaire et peuvent présenter un risque accru d'admissions en hospitalisation. Il est nécessaire d'envisager des adaptations du traitement des troubles du comportement alimentaire pour cette population. L'objectif de cette étude est d'explorer les expériences des cliniciens dans la mise en œuvre de la FT-ED auprès de jeunes autistes atteints d'anorexie mentale ainsi que les adaptations actuellement mises en œuvre. MÉTHODE : Des cliniciens formés à la FT-ED ayant de l'expérience dans la mise en œuvre de cette modalité auprès de jeunes autistes et de leurs familles ont été invités à participer à des entretiens. Les transcriptions ont été analysées au moyen de l'analyse thématique réflexive. RÉSULTATS : Onze cliniciens ont réalisé des entretiens et l'analyse a généré quatre thèmes et huit sous-thèmes : (1) Contexte systémique, (2) Évoquer un possible autisme, (3) Chevauchement entre autisme et troubles du comportement alimentaire, (4) Manuel versus adaptations. CONCLUSIONS : Cet article constitue la première exploration de l'expérience des cliniciens mettant en œuvre la FT-ED auprès de jeunes autistes et de leurs familles, et a mis en évidence des considérations propres à cette population. Il s'agit d'une première étape pour envisager des adaptations du modèle FT-ED, dans le but de rendre les traitements des troubles du comportement alimentaire plus efficaces, accessibles et acceptables pour les jeunes autistes et leurs familles.

OBJECTIVE: Eating disorder focused family therapy (FT-ED) is the leading outpatient intervention for adolescents with Anorexia Nervosa. Autistic people report poorer eating disorder treatment experiences and may be at increased risk of inpatient admissions. There is a need to consider adaptions to eating disorder treatment for this population. The aim of this study is to explore the experiences of clinicians in the delivery of FT-ED for Autistic young people with Anorexia Nervosa and any adaptations currently being implemented. METHOD: FT-ED trained clinicians who had experience of delivering this modality with young Autistic people and their families, were invited to take part in interviews. Transcripts were analysed using Reflexive Thematic Analysis. RESULTS: Eleven clinicians completed interviews and analysis generated four themes and eight subthemes: (1) Systemic context, (2) Raising potential autism, (3) Autism eating disorders crossover, (4) Manual versus adaptations. CONCLUSIONS: This paper is the first exploration of clinician's experience delivering FT-ED to Autistic young people and their families and highlighted unique considerations with this population. It is an initial step to consider adaptations to the FT-ED model, with the aim of making eating disorder treatments more effective, accessible and acceptable for Autistic young people and their families.
