Cet article examine les expériences de proches aidants familiaux soutenant leurs enfants et leurs frères et sœurs présentant des déficiences intellectuelles, de l’autisme et des troubles de santé mentale concomitants dans des services d’hospitalisation. Les connaissances relationnelles des proches aidants concernant la communication, les besoins sensoriels et les antécédents traumatiques sont souvent exclues des soins, ce qui renforce les déséquilibres de pouvoir systémiques et légitime des pratiques coercitives telles que la contention et l’isolement. Au moyen de quatre groupes de discussion communautaires en ligne réunissant vingt-sept proches aidants, animés par des cochercheurs ayant une expérience vécue, une analyse thématique a révélé les doubles rôles des proches aidants en tant que membres de la famille et défenseurs naviguant dans l’exclusion institutionnelle. Les participants ont décrit comment la détresse était mal interprétée et qualifiée de « comportement difficile », renforçant des réponses institutionnelles qui perpétuaient les préjudices et un traumatisme relationnel cumulatif pour les personnes concernées et leurs familles. Les proches aidants ont appelé à un partenariat, à des cultures adaptées aux traumatismes et à une réforme des politiques. En réponse, nous proposons le cadre de réduction des pratiques restrictives inclusif des proches aidants (CIRP) : un modèle fondé sur les droits qui intègre l’autorité des proches aidants par la reconnaissance, la réparation, la reconnexion et la réforme afin de remédier à l’injustice épistémique et de faire progresser des soins de santé mentale collaboratifs et éthiques.
